Here's the outside of my mailer. The two bottom panels receive the address labels. It's important to address the reply card as well, because some folks don't take the time to put their name and address on it. If you just put a blank for their name, you will likely get at least one or two cards back and not know where they came from.

The piece measures 6" x 12.625". The lower part (the reply card) is perforated with a simple device used for marking patterns, found in fabric stores for about 99 cents. It's just a little serrated metal wheel with a plastic handle.

The reply card gets a 20 cent stamp and the mailer, when folded and sealed with a label, gets a 33 cent stamp.

And this is what's on the inside. The lower panel, again, is the perforated reply card.
